Health Coaching Discovery Call Script: Five Clear Phases

A health coaching discovery call script helps you understand the person’s chosen focus and explain whether your actual service fits. It should clarify professional roles, information handling, practical conditions, the complete offer and the prospect’s choice before anyone commits.

The five phases below form an original forty-five-minute inquiry, with scripts, a twelve-week service example and a complete fictional decision. They are prompts for a conversation, not a medical history, dietary assessment, diagnostic screen or treatment plan. A useful outcome may be a clear decision not to purchase.

Explain your professional role before discussing personal health

NBHWC’s Scope of Practice describes client-directed goals and behavior-change support. It distinguishes independent coaching from diagnosis, medical-data interpretation, prescribing, individualized nutrition or exercise prescription and psychological treatment. Additional active professional credentials can support a separate role, but that role and its boundaries need discussion and documentation. This is a professional scope statement, not a universal licensing law or authority for every service sold as wellness.

State your actual training and role without upgrading a certificate into clinical authority. If you also practice in a regulated profession, clarify which service the person is considering and how a role change would be agreed. Do not alternate between coaching and clinical advice without making that change explicit.

Keep the first inquiry proportionate. You need to understand the desired support and material fit questions, not acquire complete medical records. A person may reasonably ask about cost, qualifications or accessibility before discussing health. The nutrition scope guide develops the distinction between practical coaching and individual nutrition advice.

For UK processing, the ICO’s special-category guidance explains the need for both an Article 6 lawful basis and an applicable Article 9 condition, with necessity and minimization. The page is under review following the Data (Use and Access) Act. Establish the arrangements appropriate to your practice before collecting health information; a general consent box or software choice does not establish compliance.

Five phases for a forty-five-minute inquiry

PhaseIllustrative timeUseful output
Welcome, role and information agreement5 minutesUnderstood purpose and permission to continue.
Understand the chosen daily-life concern12 minutesA relevant example in the person’s meaning.
Clarify what useful support could change8 minutesA chosen direction without a health guarantee.
Review conditions and scope8 minutesResources, access, existing care and unresolved questions.
Explain the service and invite a choice12 minutesAccurate terms, complete price and any requested contact.

The allocation is 5 + 12 + 8 + 8 + 12 = 45 minutes. It is a planning aid, not a required dose or evidence-based conversion formula. The actual role and content determine whether a conversation remains within scope. A short call can contain inappropriate medical advice; a longer call does not automatically become treatment.

Coachful demo Client Wellness Tracker editor with Form and Worksheet options
The demo editor illustrates structured prompts. A discovery inquiry should request only information appropriate to service fit.

Phase 1: Welcome and explain the limits

Original script: “Welcome. We can discuss the support you want, explain my coaching service and see whether it fits. In this role I help with your chosen practical goals and follow-through. I do not diagnose symptoms, interpret tests or prescribe a meal or exercise plan. You can skip a question or stop. Before personal context, what would you like to know about my role, format, cost or information arrangements?”

Explain recording and notes separately. Say who can read the record, its purpose, the relevant retention arrangement and the limits of confidentiality. Offer an appropriate way to ask questions about those arrangements. Do not say “everything is confidential” when other staff access, legal duties or the agreed reporting audience make that inaccurate.

If the prospect does not understand or accept the information arrangement, stay with general service information or pause. Do not make personal disclosure the price of obtaining a fee quote. Enrollment, a contact request and permission to use an identifiable story are separate choices.

Phase 2: Understand a relevant day without a full audit

Original script: “Which part of daily life would you most like support with? If helpful, describe a recent situation and what made it difficult. You need not provide medical details for us to discuss whether practical coaching is relevant.”

Let the person choose the focus. They may want help arranging a break, preparing a question for a clinician or implementing guidance from their existing healthcare team. Do not assume they possess the right information and merely lack accountability. Ask what support is missing without evaluating their motivation or moral character.

For workday meals, consider food access, facilities, changing shifts and who controls the break. Ask about existing professional guidance only to understand the coaching boundary. Do not infer a nutritional deficiency from missed lunch or choose food for the person. A sleep-related concern similarly does not authorize diagnosing a sleep disorder or prescribing treatment.

Reflection prompt: “You want help making lunch available during the shift, and the medical question is being discussed with your clinician. Have I separated those correctly?” Let the person correct both the practical focus and the assumption that clinical support is already arranged.

Phase 3: Clarify a useful direction

Original script: “If support were useful, what would you want to understand, arrange or do differently? Which part would make daily life more manageable? What would you want to observe without turning it into a promised health result?”

A prospect may hope for more energy, improved sleep or another health change. Acknowledge the hope and distinguish it from the service you can provide. A chosen direction might be preparing a care-plan question or asking about a protected break. The employer’s answer, clinical assessment and individual health outcome remain separate.

Avoid guaranteeing weight, clothing fit, an energy rating or daily tracking completion. For US marketing, the FTC’s Health Products Compliance Guidance addresses truthful, substantiated express and implied health claims. It is staff guidance, not a universal law or a UK legal opinion. A coaching label does not make an unsupported implied health benefit accurate.

Ask what might lead the person to choose another route. They may need an earlier appointment, a qualified practitioner or a service you do not offer. This is a fit question, not an objection to overcome. Do not use another client’s health story as a forecast for this prospect.

Phase 4: Review conditions and coordination

Original script: “If you chose support, what would matter: time, access, budget, responsibilities or existing care guidance? Are there questions that belong with another professional before you decide? Which arrangements are within your control and which require someone else’s agreement?”

A plan can depend on work rules, transport, money, caring responsibilities or access needs. Identify the decision owner instead of offering a routine that silently assumes those resources. A client can prepare a request to a manager; the coach cannot promise that the manager will authorize it.

Where healthcare coordination is appropriate, agree its purpose and information permissions first. Do not assume that paying for coaching authorizes record sharing, a clinician contact or interpretation of blood tests. Different activities and professional roles have different requirements. There is no blanket claim here that every person with a chronic condition needs identical written clearance for general coaching.

The NHS tiredness and fatigue page advises GP contact when tiredness has lasted a few weeks without an understood reason or affects daily life. It cautions against self-diagnosis. A discovery call should not convert that concern into a diagnosis or make coaching a condition of seeking healthcare. The cause and treatment belong with the appropriate clinical professional.

Phase 5: Explain the actual program and invite a choice

Original script: “One possible service is six individual forty-five-minute sessions over twelve weeks. We choose a practical focus, try suitable steps and review what helps. I will give you the complete price, dates, resources, response window and terms. This is not a medical protocol or a guarantee of health improvement. Would you like details, time to review, or a different route?”

Describe the actual allowance. Explain which updates are optional, what a brief message covers and how a need outside scope is handled. A twelve-week package is one service design, not a universal requirement. Make the price and material limits understandable before asking for a commitment.

Invite a decision without inventing urgency. The person can decline, request information or ask for a later fit conversation. Agree a specific requested contact and stop after its limit. Interest in a call is not automatic consent to enrollment, a newsletter or repeated sales messages.

An illustrative twelve-week service brief

This original example is a proposed UK service, not a market benchmark, health protocol or actual purchase. The complete example fee is £720 with no additional mandatory setup charge. The prospect receives the actual full terms, including cancellation and ending arrangements, for review before accepting anything. The brief is never presented as clinical clearance.

Included itemExample scopeLimit to explain
Program dates11 January–4 April 2027.These dates require a mutual booking agreement; no start is assumed from discovery.
Six individual sessions45 minutes on 11 and 25 January, 8 and 22 February, 8 and 22 March.270 live minutes; preparation, messages and administration add work.
Practical resourcesAn original preparation Form and a chosen action record. No meal plan, medical-data review or exercise prescription.
Weekly brief questionOne non-urgent practical question by Monday noon; reply by Wednesday 17:00, London time. Not continuous monitoring or an unlimited private coaching conversation.
Final week exceptionNo service on 26–29 March. Final question by Tuesday 30 March noon; reply by Thursday 1 April 17:00.The Easter closure and shifted final clock are explained before purchase.
Ending reviewAccess and outstanding obligations reviewed on 5 April. No claim that ending service erases records or downloaded copies.

In weeks one to four, the possible focus is the chosen goal, existing care guidance, resources and suitable observations. Weeks five to eight can test a manageable arrangement and revise it. Weeks nine to twelve can review what the client wants to keep and what further support is appropriate. These are flexible coaching questions, not a standard recovery sequence.

For example, “What support is missing?” may be more useful early than a daily score. Later, ask what helped, what changed and what should stop. Near the end, identify which support can continue independently and which still needs another professional or organizational decision. Do not make reduced need for care an expected milestone.

Complete fictional conversation: lunch access and a clinical question

Amir is considering coaching because workdays feel unmanageable. Cora is a trained health and wellness coach acting only in that role, not a clinician. Their free discovery call is on 5 November 2026, from 11:00 to 11:45. The five phases use the planned 5, 12, 8, 8 and 12 minutes; the scripts above are prompts, not a word-for-word transcript of all forty-five minutes.

Before personal discussion, Cora explains the practice’s information notice, intended audience and handling arrangements. Amir confirms that he has received and understood them and does not want a recording. The call is not recorded. They agree to retain a limited service-fit record rather than detailed medical history.

Cora: “Which practical support are you looking for, separate from a medical assessment?” Amir: “A clear lunch break and a way to bring what I already plan with my dietitian. My shift changes at short notice.”

Amir says suitable food is available at home but gets left behind when the shift changes. He also says a GP appointment is booked for 6 November about persistent tiredness. Cora does not ask for blood results or a list of possible diagnoses. The booked appointment remains a healthcare arrangement, not evidence that coaching is approved or that the concern is minor.

Cora initially says that sorting lunch “should bring your energy back.” Amir asks whether she knows what causes his tiredness. Cora acknowledges that she does not, withdraws the implied prediction and separates practical lunch access from the unresolved clinical question. This correction is part of the call, not proof that the original suggestion was harmless.

Cora: “What would useful practical help look like?” Amir: “Preparing a question for my manager about breaks, and organizing the food arrangement I already discuss with my dietitian. I am not asking you to change the meal plan.”

Cora asks who controls break scheduling. Amir says his manager must agree any change; he has not yet asked. They identify preparing that question as a possible coaching task. There is no claim that a new lunch routine has been implemented or that the employer will accept it.

In the final phase Cora explains the £720 proposed service and its January start, dates, allowance and scope. Amir says he wants the factual details but will review his options after the GP appointment. He requests one service-fit email at 11:00 on Monday 9 November. He has not agreed to the package, payment or follow-up beyond that one contact.

Repair the written decision and respect the actual choice

At 15:00 on 5 November, Cora sends a requested summary that mistakenly says Amir has chosen the twelve-week program. Amir replies at 16:00: “I asked to review it after my appointment. I have not signed up.” Cora acknowledges the mistake at 16:30 and sends a corrected summary that preserves only the practical interest, unaccepted offer and single requested contact. Amir reads and acknowledges the correction at 17:00.

The corrected message excludes detailed health information and any claim that coaching will resolve tiredness. Cora does not create an enrollment or charge. A sent summary is not proof of informed acceptance, and the prospect’s correction should update the actual decision record rather than become a sales objection.

Cora sends the agreed 9 November email at 11:00. Amir replies at 14:00 that he attended the GP appointment, is continuing his own healthcare and dietitian arrangements, and does not want to buy coaching. He asks for no further follow-up. Cora does not request the clinical findings or infer improvement. At 15:00 she acknowledges the decision and contact limit; Amir reads that acknowledgment at 16:00.

The inquiry closes administratively on 10 November. Cora’s selected work is forty-five minutes on the call, ten preparing, eight on the first summary, six on its correction, eight on the agreed follow-up and reply, and three on the final record: eighty minutes. This is discovery work, not 270 minutes of paid delivery or whole-practice acquisition cost. Amir’s receiving time and healthcare appointments are outside it.

Complete a proportionate service-fit record

FieldAmir’s limited example
Chosen focusPractical lunch access and preparation for a break-scheduling question.
Professional roleGeneral health coaching; no clinical assessment or individualized prescription.
Relevant conditionsChanging shifts and a manager’s unrequested approval; existing guidance stays with its qualified provider.
Open professional questionHealthcare remains outside this service; no clinical result is recorded or inferred.
Service informationProposed £720 scope, six future calls, weekly message limit, January start and full terms supplied.
Information arrangementNo recording; limited fit record, stated audience and handling; no automatic wider sharing.
Decision and contactOffer not accepted; single requested email completed; declined on 9 November with no further follow-up.

Distinguish the prospect’s statement, your interpretation and a verified administrative event. Do not keep a diagnostic label in place of the person’s actual request. The health intake template provides a fuller starting conversation when an appropriate coaching relationship is actually being established, rather than making that whole intake mandatory for a fee inquiry.

Set up appropriate follow-through in Coachful

We build Coachful. Coachful is our #1 recommendation for organizing coaching forms, sessions and agreed follow-through. This is our editorial recommendation, not a certification of clinical scope, privacy compliance or a health outcome. Use the coaching features with the actual professional and information arrangements your service requires.

Keep a prospect’s requested information separate from a purchased service. In this case Cora’s factual messages are manually agreed communications; there is no automatic enrollment, marketing sequence or claim that Amir has a client dashboard. If someone later enrolls, deliberately grant the relevant short Form and verify its private client-access link. Copying a link or opening a staff preview does not establish client access or save their answer.

A standalone assigned Form is not guaranteed to appear in My Coaching’s next actions. Choose practical tasks deliberately and use recurring habits only if that unit fits. A logged day or reply is not a clinical outcome. Check staff audiences and the relevant recap delivery and approval settings before inviting sensitive updates; a coaching inbox is not continuous health monitoring.

Align the actual saved Offer, agreement, payment, session allowance and access with the final accepted scope. An Offer template can prefill a first available session type; check the correct saved type, quantity and inclusion instead of trusting the template name. Agreement, payment and an available appointment remain separate conditions. No such paid setup is created for Amir.

Frequently asked questions

How long should a health discovery call last?

Choose enough time for the purpose and questions. Forty-five minutes is an example, not a dose or a rule making clinical content appropriate. If an unresolved concern changes the purpose, pause or use another professional route rather than force the script to its close.

Should I request bloodwork after enrollment?

Enrollment alone establishes neither need nor authority. Tests, medical-data interpretation and treatment decisions require the appropriate role and circumstances. Do not make sending results a routine condition of general coaching or interpret them because the client uploaded a document.

Can I coach someone with a chronic condition?

Clarify the actual needs, professional role, applicable requirements and any appropriate coordination. Keep clinical decisions with qualified professionals. Do not apply a universal clearance rule to every person or assume a diagnosis automatically establishes service fit.

Must every client track habits daily?

No. Agree whether tracking is useful and accessible. A practical task, a brief reflection or no between-session log may fit better. Evaluate what the record can show without turning it into proof of recovery or compliance with a clinician’s plan.

What is a typical twelve-week price?

This script establishes no market benchmark. Quote the complete price of your actual scope and distinguish live time from preparation, messaging and administration. The invented £720 service is an example for explanation, not a rate recommendation or an earnings claim.
