What to ask on a coaching intake form
Basics and logistics (time zone, best contact method), the goal in their words, what they have already tried, what is at stake if nothing changes, and what they expect from you. Add a one-to-ten scale for the thing you will track, so you have a baseline from day one.
Keep it under 20 questions. Every question you add costs completion rate. If you need more, split it: a short form before booking, a longer one after the first session.
Intake forms versus discovery calls
The form collects facts and a baseline; the discovery call reads the person. Send the form after the call is booked and before it happens, so you walk in prepared and spend the call on the conversation rather than the paperwork.

What questions should be on a coaching intake form?
Contact and logistics, the goal in the client's words, history and what they have tried, obstacles, expectations of coaching, a readiness or confidence scale, and consent to your terms. The generator tailors these to your niche, for example health history for health coaches or leadership context for executive coaches.
When should I send the intake form?
After the discovery or first session is booked and before it happens. Sending it before booking lowers conversion; sending it after the first session wastes the session on basics.
How long should a coaching intake form be?
Ten to twenty questions, five minutes to complete. Longer forms get abandoned. Put the essential questions first so a partial submission is still useful.
